First Fatal Human Case of Beta-Cyfluthrin Intoxication in Türkiye: A Case Report

Introduction Cyfluthrin, a type II synthetic pyrethroid, is extensively used in agricultural and veterinary practices. While generally considered to have low mammalian toxicity, significant exposure to concentrated formulations can lead to systemic toxicity and multiorgan failure. Human fatalities due to cyfluthrin are rare and not well-characterized in the literature. Case Report We report the case of a 67-year-old female shepherd with no known comorbidities who presented to the emergency department approximately 24 hours after dermal and possible inhalational exposure to a veterinary insecticide containing 25 g/L beta-cyfluthrin in emulsifiable concentrate form. She had applied the formulation directly to livestock without protective equipment. Within hours, she developed acute retrosternal chest pain, vomiting, and transient loss of consciousness. Upon arrival to the emergency department, she was hypotensive, bradycardic, and hypoxic. Laboratory analysis revealed severe metabolic acidosis, hyperglycemia, markedly elevated hepatic and cardiac enzymes, acute kidney injury, and coagulopathy. Imaging showed a moderate pericardial effusion without tamponade. Despite intensive supportive care, including high-dose sodium bicarbonate, N-acetylcysteine, and fluid resuscitation, her condition deteriorated, culminating in disseminated intravascular coagulation, shock, and death on ICU day 8. Conclusion This case illustrates the potential lethality of beta-cyfluthrin exposure in humans and underscores the importance of early recognition, protective practices, and aggressive management in suspected pyrethroid poisoning.

Türkiye'de İlk Ölümcül Beta-Siflutrin Zehirlenmesi Vakası: Bir Vaka Sunumu

Öz

Giriş Cyfluthrin, Tip II sentetik bir piretroid olup tarımsal ve veterinerlik uygulamalarında yaygın olarak kullanılmaktadır. Genellikle memeliler için düşük toksisiteye sahip olduğu düşünülse de, yoğun formülasyonlara maruz kalma durumunda sistemik toksisite ve çoklu organ yetmezliği gelişebilir. Cyfluthrin'e bağlı insan ölümleri nadirdir ve literatürde iyi tanımlanmamıştır. Vaka Raporu Bu yazıda, bilinen herhangi bir eşlik eden hastalığı olmayan 67 yaşında kadın bir çobanın, 25 g/L beta-cyfluthrin içeren emülsifiye konsantre formundaki bir veteriner insektisitine dermal ve muhtemel inhalasyon yoluyla maruz kalmasını takiben acil servise başvuru süreci sunulmaktadır. Hasta, formülasyonu koruyucu ekipman kullanmaksızın doğrudan hayvanlara uygulamıştır. Saatler içerisinde retrosternal göğüs ağrısı, kusma ve geçici bilinç kaybı gelişmiştir. Acil servise başvurusunda hipotansif, bradikardik ve hipoksemikti. Laboratuvar incelemelerinde ağır metabolik asidoz, hiperglisemi, belirgin derecede yükselmiş karaciğer ve kalp enzimleri, akut böbrek hasarı ve koagülopati saptanmıştır. Görüntülemede tamponat oluşturmayan orta düzeyde perikardiyal efüzyon görülmüştür. Yüksek doz sodyum bikarbonat, N-asetilsistein ve sıvı replasmanını içeren yoğun destek tedavisine rağmen durumu kötüleşmiş, yaygın damar içi pıhtılaşma (DİK), şok ve yoğun bakımın 8. gününde ölümle sonuçlanmıştır. Sonuç Bu olgu, beta-cyfluthrin maruziyetinin insanlarda potansiyel ölümcüllüğünü ortaya koymakta ve piretroid zehirlenmelerinde erken tanı, koruyucu önlemler ve agresif tedavi yaklaşımlarının önemini vurgulamaktadır.
