Laparoscopic Surgical Staging in Gynecologic Cancers: Nodal Adequacy and Perioperative Outcomes in 105 Patients

Öz

Aim: To evaluate the feasibility and nodal adequacy of laparoscopic surgical staging for gynecologic malignancies at a tertiary gynecologic oncology center, with particular attention to perioperative tolerability in an obese population. Materials and Methods: This single-center retrospective study included 105 consecutive patients who underwent laparoscopic surgical staging for histologically confirmed gynecologic malignancy over a 24-month period. Demographic, clinicopathologic, surgical, and perioperative hematologic data were extracted from institutional records. Surgical adequacy was defined a priori as a pelvic lymph node count ≥10 and a paraaortic lymph node count ≥5. Results: The median age was 62 years (IQR 55–66) and the mean body mass index was 32.6 ± 5.2 kg/m²; 68.6% of patients were obese. Endometrial cancer accounted for 93.3% of cases, and 88.6% had early-stage (FIGO I–II) disease. The median pelvic nodal yield was 18 (IQR 13–23), with adequacy achieved in 95.2% of patients; paraaortic lymphadenectomy was performed in 68.6%, all meeting the adequacy threshold. Nodal metastasis was detected in 10.5%, and nodal yield was preserved across body mass index strata (ρ = 0.166, p = 0.091). The median 24-hour hemoglobin drop was 1.1 g/dL (IQR 0.8–1.6), with a drop of ≥2 g/dL in 14.3%. During a median follow-up of 8 months, recurrence occurred in two patients (2.0%). Conclusion: Laparoscopic surgical staging for gynecologic malignancies achieved high rates of nodal adequacy with limited perioperative hematologic impact, supporting its routine use in tertiary gynecologic oncology practice, including in obese populations.
