The role of fecal occult blood test in the early diagnosis of Necrotizing Enterocolitis

Aim: It was aimed to contribute to the relevant literature by evaluation the relationship between the positive or negative stool occult blood test result studied in the premature infants with necrotizing enterocolitis (NEC) and birth week, birth weight and NEC stage. Method: Patients diagnosed with NEC and treated at Mersin University Faculty of Medicine Neonatal Intensive Care Unit between January 2018 and January 2024 were included in the study. Occult blood in the stool was examined at the time they were diagnosed with NEC. The diagnosis and staging of NEC was made according to the modified Bell classification. The relationship between the occurrence of occult blood in the stool and gestational age and birth weight was examined with the Independent T-test. In addition, the relationship between the presence of occult blood in the stool at the time of initial diagnosis and the stages of NEC was examined with the chi-square test. Results: No significant relationship was detected between the presence of occult blood in the stool and the NEC stages, gestational birth weeks and birth weights of the patients included in the study (p=0.158), (p=0.339), (p=0.559). Conclusion: It is thought that the relationship between fecal occult blood examination and markers such as NEC stage, gestational age and birth weight is weak in NEC.

Nekrotizan Enterokolit’in Erken Tanısında Gaitada Gizli Kan Tetkikinin Yeri

Öz

Amaç: Nekrotizan enterokolit (NEK)’li prematüre infantlarda çalışılan gaitada gizli kan testinin pozitif veya negatif olmasının doğum haftası, doğum kilosu ve NEK evresi ile olan ilişkisinin değerlendirilmesi yolu ile ilgili literatüre katkı sağlanması amaçlanmıştır. Yöntem: Çalışmaya Ocak 2018- Ocak 2024 tarihleri arasında Mersin Üniversitesi Tıp Fakültesi Yenidoğan Yoğun Bakım ünitesinde NEK tanısı ile tedavi edilen hastalar dahil edildi. NEK tanısı aldıkları dönemde gaitada gizli kan görülme durumları incelendi. NEK tanısı ve evrelemesi modifiye Bell sınıflamasına göre yapıldı. Gaitada gizli kan görülme durumları ile gestasyonel doğum haftaları ve doğum kiloları arasındaki ilişki İndipendent T-test ile incelendi. Ayrıca ilk tanı anında gaitada gizli kan görülme durumu ile NEK evreleri arasındaki ilişki ki-kare testi ile incelendi. Bulgular: Çalışmaya dahil edilen hastaların NEK evreleri, gestasyonel doğum haftaları ve doğum kiloları ile gaitada gizli kan görülme durumu arasında anlamlı bir ilişki tespit edilmedi (p=0.158), (p=0.339), (p=0.559). Sonuç: NEK’te gaitada gizli kan tetkikinin NEK evresi, gestasyonel doğum haftası ve doğum kiloları gibi belirteçler ile ilişkisinin zayıf olduğu düşünülmektedir.
