Nikotin bağımlılığını belirlemede Fagerström Nikotin Bağımlılık Testinin (FBNT) değerlendirilmesi ve nikotin bağımlılığı için yeni bir test oluşturulması

Öz

GİRİŞ ve AMAÇ: FBNT nikotin bağımlılığının değerlendirilmesinde oldukça yaygın kullanılan 6 soruluk testtir. Bu çalışmada FBNT’nin nikotin bağımlılığını belirlemekteki gücü değerlendirilmiş, yeni bir test oluşturulmaya çalışılmıştır.YÖNTEM ve GEREÇLER: Metodolojik bir çalışmadır. Sigara bırakma merkezine başvurmuş 640 kişi çalışma grubunu oluşturmaktadır.Bağımlılığın göstergesi olarak sigara bırakma merkezine başvurmasına, tedaviye alınmasına rağmen izlem sonunda kişinin sigarayı bırakamaması alınmıştır. Tüm olgulara başvuru sırasında FBNT ve sigara bağımlılığı ile ilgili olabileceği düşünülen soruların yeraldığı anket uygulanmıştır. FBNT ve anketteki her bir sorunun sigara bırakmayla ilişkisi incelenmiştir. Sigarayı bırakmayla ilişkisi (korelasyonu) yüksek sorular belirlenip, bu sorularla yeni test oluşturulup FBNT ile karşılaştırılması yapılmıştır.İstatistiksel analizlerde Korelasyon Analizi, t ve ki-kare testleri kullanılmıştır. BULGULAR: Çalışma grubumuzdaki 640 kişinin yaş ortalaması 41,4 ve %71’i erkektir. Ortalama 20 aylık izlem sonucu sigara bırakma oranı %21,5’dir. FBNT’nin her bir sorusuyla sigara bırakma oranları arasında ilişki incelendiğinde 4 sorunun oldukça ilişkili olduğu buna karşın 2 sorunun ise zayıf ilişkili olduğu tespit edilmiştir. Ayrıca “Sigara içme nedenleri anketi”nde yer alan sorular ile sigara bırakma arasında ilişki incelenmiş, en çok ilişki saptanan 3 soru belirlenmiştir. Bu üç soru FBNT’nin etkili olduğu saptanan 4 sorusuna ayrı ayrı eklenerek 3 farklı model oluşturulmuştur. Bu modeller içinden ‘‘Konuşurken, sohbet ederken, çayla birlikte canım bir sigara ister’’ sorusunun eklenerek oluşturulan modelin bağımlılığını hem diğer modellere hem de FBNT’ye göre daha iyi gösterdiği görülmüştür. TARTIŞMA ve SONUÇ: Oluşturulan 5 soruluk testin nikotin bağımlılığını daha iyi gösterdiği gözlenmiştir.

Assessment of the Fagerström Nicotine Dependant Test (FNDT) used in the determination of nicotine dependence and developing a new test for the nicotine dependance

INTRODUCTION: FNDT is a 6-item questionnaire, which is quite commonly used in determining nicotine dependence level. In this study, the effectiveness of FNDT was analyzed and the researchers attempted to develop a new test.METHODS: This is a methodological study. Totally 640 participants who admitted to smoking cessation center form the study group. As an indicator of the nicotine addiction level, though they applied to the smoking cessation center, “being unable to quit smoking” was considered. Firstly,a questionnaire including both FNTD and questions about smoking addiction were applied to cases in the application process.The most efficient items in indicating nicotine addiction were determined among both FNDT questions and the additional questions.A new test was created with these questions and compared with FBNT.Correlation analysis, t and chi-square tests were used in analysis of data.RESULTS: The average age of a total of 640 patients is 41.4 and 71% of whom are male. The rate of quitters after approximately 20-months monitoring period is 21.5%. When the relationship between each of FNDT questions and “being unable to quit” was investigated, four questions were found to be highly related but two questions of FBNT were not found to be related. The relationship between the additional questions about smoking and “being unable to quit smoking” was also investigated, and three questions were found to be highly related. These three questions were added separately to four questions of FBNT and three different models were developed.The model obtained by adding the question “I'd like a cigarette when I am confabing or drinking tea” was found to be more efficient in determining the level of addiction according to both FBNT and other two models.DISCUSSION AND CONCLUSION: Nicotine addiction level can be determined better with this newly revised 5-item questionnaire.
