Skafoid kırıklarının vida ile fiksasyonu sonrasırehabilitasyon sonuçları: retrospektif çalışma

Öz

AMAÇ: Skafoid kırığıperkütan vida fiksasyonu sonrasırehabilitasyon sonuçlarının sunulması, uygulanan rehabilitasyon programının fonksiyonel durum, yaşam kalitesi ve hasta memnuniyetine etkisinin araştırılması amaçlandı.YÖNTEMLER: Skafoid 1/3 orta bölge kırığıiçin minimal invaziv perkütan vida fiksasyonu yapılmışolan hastaların son durumlarıdeğerlendirildi. Hastaların demografik, klinik özellikleri, operasyon sonrasısüre, operasyon sonrasıev egzersizi veya fizik tedavi programıuygulama hikayesi kaydedildi. Aktivite ve istirahat ağrı şiddeti (Görsel Analog Skala), el bileklerinin son durumundan memnuniyet düzeyi (GAS), el bileği eklem hareket açıklığıve kas güçleri (kaba kavrama, uç, lateral ve üçlü tutma) değerlendirildi. Fonksiyonel değerlendirme kısa form-Disabilities of arm, shoulder and hand anketi (Q-DASH) ve Mayo el bileği skoru ile, yaşam kalitesi değerlendirmesi Kısa form-36 (KF-36) ile yapıldı. BULGULAR: Ortalama yaşı26,3±6,3 yıl olan, 23 erkek (%95,8), 1 kadın (%4,2) hasta değerlendirildi. Postoperatif ortalama 6. ayda değerlendirilen hastaların ortalama istirahat ağrısı şiddeti 0,9±1,2 (0-4), aktivite ağrısı şiddeti 3,6±2,0 (0-8) gibi düşük derecede, el bileğinin son durumundan memnuniyetleri ortalama 8,3±1,5 (5-10) ile iyi düzeydeydi. Q-DASH skorlarıortalama 13,3±13,7 puanla oldukça iyiydi ve Mayo skorlarımükemmel iyileşme düzeyindeydi. Sadece ev egzersizi alan hastalar ile ev egzersizine ek olarak fizik tedavi programıalmışolan hastalar arasında operasyondan ortalama 6 ay sonra değerlendirme parametreleri açısından anlamlıfark olmadığısaptandı.SONUÇ: Skafoid kırıklarının perkütan vida fiksasyonu sonrası, prospektif randomize kontrollü çalışmalar ile ev egzersiz programının sonuçlarının klinikte uygulanan fizik tedavi programıile karşılaştırılması, etkin ve ekonomik rehabilitasyon yönteminin belirlenmesinde faydalıolacaktır.

Rehabilitation results after screw fixation of scaphoid fractures: retrospective study

OBJECTIVE: The aims of the study were to represent the rehabilitation results after screw fixation of scaphoid fractures, and to explore the effects of applied rehabilitation programme on functional status, quality of life, and patient satisfaction.METHODS: The prognosis of the patients who had operated with minimal invasive screw fixation for the middle third of the scaphoid fracture were evaluated. Demographic, and clinical features of patients, postoperative period, home exercise or physical therapy application after operation were recorded. Activity and resting pain level (Visual analog scale), patient satisfaction with the last situation of the wrist (VAS), range of motion of wrists, muscle strengths (grip strength, tip, lateral, tripod grip) were evaluated. Functional situation was assessed by Quick-Disabilities of arm, shoulder and hand and Mayo wrist score, and quality of life was evaluated by Short Form-36 (SF-36)RESULTS: Twenty three male (95.8%), one female (4.2%) patients with the mean age of 26,3±6,3 years were assessed. Resting, and activity VAS was low with the level of 0,9±1,2 (0-4), and 3,6±2,0 (0-8) respectively, and patients satisfaction with the last state of wrist was good with the level of 8,3±1,5 (5-10) of the patients who were assessed at the mean postoperative time of 6 months. Q-DASH scores were considerably good with the level of 13,3±13,7, and Mayo wrist scores were at the excellent level. There were no differences between the patients who had only home exercise programme, and patients who had physical therapy programme in addition to home exercise for the evaluation parameters at 6 months after operation. CONCLUSION: Comparison of the results of home exercise programmes and phsysical therapy programmes with the prospective, randomized controlled studies after percutaneous screw fixation of the scaphoid fractures will be beneficial to detect the efficient and cost-effective rehabilitation method.
