Endometriosis at the incision site after C-section procedure

Öz

Objective: Endometriosis is defined as the presence of endometrial glands and endometrial stroma outside the uterus. In this study, we aimed to investigate the possible relationship between the number of caesarean section (C-section), size of endometriosis, age, and time to diagnosis after C-section in patients diagnosed with endometriosis after C-section.
Materials and Methods: Patients who were operated for mass formation after C-section and pathologically diagnosed with endometriosis in our clinic between December 2012 and December 2012 were retrospectively analyzed. Data including age of the patients, number of previous C-section, time to diagnosis following the procedure, size of endometriosis were recorded. The patients were divided into two groups as Group 1 consisting of nine patients with a mass on the right side of the midline and Group 2 consisting 12 patients with a mass on the left side.
Results: There was no significant difference in the age, number of previous C-section, time to diagnosis after the procedure, and size of endometriosis between the groups (p=0.795, p=0.655, p=0.432, and p=0.434, respectively). A total of 90% of the patients had pain, while 86% had a mass. In the majority of the patients, the mean time to diagnosis after the first C-section was 14.9±6.8 months.
Conclusion: Our study results suggest that endometriosis in addition to incisional hernia and hematoma should be taken into consideration in the differential diagnosis in patients presenting with pain and/or a mass which can enlarge and shrink with the menstrual cycle under the incision scar after C-section.

Sezaryen sonrası kesi yerinde saptanan endometriozisler

Öz

Amaç: Endometrial glandların ve endometrial stromanın uterus dışında saptanmasına endometriozis denir. Çalışmamızda sezaryen sonrası endometriozis tanısı alan olgularda; sezaryen sayısı, endometriozis boyutu, yaş ve sezaryen sonrası geçen sürenin etkileri araştırıldı.
Materyal-metot: Kliniğimizde 2012 ocak -2018 aralık ayı arasında sezaryen sonrası oluşan kitle nedeniyle ameliyat edilip patolojik olarak endometriozis tanısı konulan olgular retrospektif olarak araştırıldı. Hastaların; yaş, önceki sezaryen sayısı, sezaryen sonrası tanıya kadar geçen süre ve endometriozis boyutları kayıt edildi. Hastalar, kitlesi orta hattın sağında olan 9 olgu grup 1, solunda olan 12 olgu ise grup 2 olarak ayrıldı.
Bulgular: İki grup arasında; yaş, önceki sezaryen sayısı, sezaryen sonrası tanıya kadar geçen süre ve endometriozis boyutu açısından anlamlı fark saptanmadı (sırasıyla; p=0.795, p=0.655, p=0.432, p=0.434). Olguların 90% ında ağrı, 86% sında ise kitle şikayeti mevcuttu. Olguların çoğunluğu ilk sezaryen sonrası, ortalama 14.9± 6.8 ay sonra saptandı.
Sonuç: Çalışmamızda sezaryen sonrası insizyon skarının altında menstruel siklusla büyüyüp küçülebilen ağrı ve/veya kitle nedeniyle başvuran hastaların tanısında insizyonel herni ve hematom dışında endometriozisinde dikkate alınması gerektiğini vurgulamak istedik.
