‘ALL-ON-FOUR ‘ , ‘ALL-ON-FIVE’ ve ‘ALL-ON-SIX’ KONSEPTi KULLANILARAK ÜRETİLEN PROTEZLERİN STRES DAĞILIMININ SONLU ELEMANLAR ANALİZ YÖNTEMİ İLE İNCELENMESİ

Öz

Amaçlar: Bu çalışmanın amacı; tam dişsiz üst çenede All-on-Four, All-on-Five ve All-on-Six tekniğine göre yerleştirilmiş implantlar üzerine yapılan protezlerin 3 boyutlu modellerinin kemik, implant elemanları ve protetik elemanlar üzerinde oluşturduğu stresleri sonlu elemanlar analiz yöntemi ile değerlendirmektir.
Gereç ve Yöntem: Tamamen dişsiz atrofik maksillada, All-on-Four, All-on-Five ve All-on-Six konseptine göre üç farklı tedavi planı oluşturulmuştur. En posterior implantlar bütün gruplarda premolarlar bölgesinde, distale doğru 30 derece eğimli olarak konumlandırılmıştır. Alt yapı; krom-kobalt (Cr-Co) alaşımından kesilmiş diş formunda, üst yapı ise monolitik zirkonya olarak ayrı ayrı modellenmiş, daha sonra kontakt noktalarından temas sağlanmıştır. Protez kantilever uzunluğu 10mm olarak sabit tutulmuştur. Kuvvetler; palatobukkal yönde 45° açıyla, 3,4,5,6 no’lu dişlere sırasıyla; 100N, 150N, 150N, 200N olacak şekilde çift taraflı olarak uygulanmıştır. Von Misses, maksimum principal stres ve minimum principal stresler elde edilmiştir.
Bulgular: All-on-Six konsepti sırasıyla kortikal kemikte, implant ve protetik elemanlar üzerinde ve spongiyöz kemikte daha düşük von Mises, maksimum principal ve minimum principal stres göstermiştir. Sonlu elemanlar analizinin sonuçlarına göre bakılan tüm parametrelerde protez altyapısındaki stress ve implantlardaki stres dışında en fazla von Misses stresine sahip olan konsept All-on-Four olarak görünmektedir.
Sonuçlar: İmplant sayısı arttıkça implant elemanları ve protetik elemanlarda görülen stres azalmıştır ancak araştırılan 3 tedavi konseptinde de stres değerleri kemik direnci sınırlarını aşmadı. All-on-Six tedavi konseptinin, atrofik maksillanın rehabilitasyonunda All-on-Four ve All-on-Five konseptlerine göre daha iyi biyomekanik davranış sergilediği söylenebilir.
