Incident Reporting in Hospitals And Its Relationship With Patient Safety Culture

Öz

Safety is an essential feature of quality healthcare. Incident reporting is an important tool in supporting patient care and ensuring patient safety. This cross-sectional study was conducted to determine the current status of incident reporting in hospitals and to examine the relationship between incident reporting (OR) and patient safety culture (PSC). Study sample included 2,055 physicians and nurses working in 9 hospitals with different status in Istanbul. The data were collected using a personal information form and the Patient Safety Culture Scale. In addition, the participants were asked whether they had reported any incidents related to patient safety in the last year. It was observed that 33.5% of the study group reported at least one incident in the last year, and the distribution of the number of reported incidents differed between accredited and non-accredited hospitals (p<0.001). It was found that nurses reported more incidents than physicians and incident reporting was lower in public hospitals than in other hospitals (p<0.001). Results of multiple regression analysis indicated that the number of reported incidents was not a strong predictor of the PSC score. However it difficult conclude this lack of relationship between incident reporting and patient safety culture as that incident reporting has no role in ensuring patient safety due to the cross-sectional nature of our study. Cross-sectional studies cannot fully reveal causal relationships as they reflect only a certain period of time. Follow-up studies are studies in which the time-dependent changes of events are examined and data are collected by repeated measurements at different times.
In this sense, follow-up studies should be conducted to clarify a cause-effect relationship, and the findings obtained in this study are instructive for follow-up studies.

Olay Raporlamanın Hastanelerdeki Durumu Ve Hasta Güvenliği Kültürü İle İlişkisi

Öz

Güvenlik, kaliteli sağlık hizmetlerinin temel bir özelliğidir. Olay raporlama, hasta bakımının desteklenmesinde ve hasta güvenliğinin sağlanmasında önemli bir araçtır. Hastanelerde olay raporlamanın mevcut durumunu saptamak ve olay raporlamanın (OR), hasta güvenliği kültürü (HGK) ile ilişkisini incelemek amacıyla yapılan kesitsel nitelikteki bu çalışmada, İstanbul’da farklı statüdeki 9 hastanede çalışan 2.055 hekim ve hemşire yer almıştır. Araştırma verileri, kişisel bilgi formu ve Hasta Güvenliği Kültürü (HGK) Ölçeği kullanılarak toplanmıştır. Ayrıca, katılımcılara hasta güvenliğine ilişkin son bir yıl içerisinde raporladıkları olay olup olmadığı sorulmuştur. Araştırma grubunun %33,5’inin son bir yıl içerisinde en az bir olay raporlaması yaptığı, akredite olan ve olmayan hastaneler arasında raporlanan olay sayısı dağılımlarının farklılık gösterdiği görülmüştür (p<0,001). Hemşirelerin, hekimlere göre daha fazla olay raporladığı ve kamu hastanesinde olay raporlamanın diğer hastanelerden daha az olduğu saptanmıştır (p<0,001). Çalışmada, raporlanan olay sayısının hasta güvenliği kültürü puanı üzerinde güçlü bir belirleyiciliği olmadığı saptanmıştır. Ancak, olay raporlama ile hasta güvenliği kültürü arasındaki bu ilişkisizliği, olay raporlamanın hasta güvenliğinin sağlanmasında rolü olmadığı şeklinde değerlendirmek araştırmamızın kesitsel niteliği nedeniyle zorlaşmaktadır. Kesitsel çalışmalar, sadece belirli bir zaman dilimini yansıttığından, nedensellik ilişkilerini tam olarak ortaya koyamaz. İzlem araştırmaları, olayların zamana bağlı değişimlerinin incelendiği, verilerin farklı zamanlarda tekrarlanan ölçümlerle toplandığı çalışmalardır. Bu anlamda bir neden-sonuç ilişkisini, açıklığa kavuşturmak için izlem araştırmalarının yapılması gereklidir ve bu araştırmada elde edilen bulgular, izlem araştırmaları için yol gösterici niteliktedir.
