Akciğer kanserinde 8. TNM sınıflandırmasına doğru

Öz

Küçük hücre dışı akciğer kanseri için önerilen 8. TNM sınıflandırmasında tümör boyutunda, 5cm’ye kadar her santiminin yeniden tanımlandığı (T1a-c ve T2a,b) ve bu şekilde tümör boyutunun prognoz açısından öneminin vurgulandığı göze çarpmaktadır. Minimal invazif adenokarsinomun T1a(mi) olarak eklendiği, akciğerin parsiyel veya total atelektazisi durumunda fark gözetilmeden aynı grupta (T2) yer aldığı, diafragma invazyonunun kötü prognoz göstermesi nedeniyle bir üst kategoriye (T4) çıkarıldığı görülmektedir. Tümörün karinadan uzaklığın prognozu etkilemediği saptandığından proksimal tümörler bir alt katagoriye (T2) alınmıştır. N tanımlayıcılarında değişiklik önerilmemiştir. Metastazlarda ise organ sayısı ve metastaz sayısının yeniden tanımlanması (M1a-c) yapılan TNM değişiklikleri olarak özetlenebilir.

Toward the 8th Edition of the TNM Classification for Lung Cancer

Öz

The tumor size, recommended for 8th TNM classification for non-small cell lung cancer is defined for each centimeters upto 5 cm (T1-c and T2, b) and hence the importance of the prognosis of tumor size is being highlighted. Minimally invasive adenocarcinoma T1 (mi) is added. Without considering the difference of partial or total atelectasis of the lung both of them is placed in the same group (T2). As diaphragmatic invasion shows poor prognosis it is removed to upper category (T4) . As the distance of the tumor to carina does not influence the outcome, proximal tumors are removed to a sub-category (T2). No changes in N identifier is recommended. Finally the number of metastatic organs and number of metastases are also redefined (M1-c).
