Akut skrotal ağrı ile başvuran pampiniform pleksus ven trombozu

Öz

Pampiniform pleksus trombozu, akut skrotal ağrının nadir bir nedenidir ve sık görülen cerrahi acilleri taklit edebilir. Bu yazıda, sol inguinal ve testiküler ağrı ile başvuran, abdominal ultrasonografi ve bilgi-sayarlı tomografi bulguları normal olan, şişlik olmaksızın lokalize hassasiyeti bulunan 62 yaşında bir erkek hasta sunulmaktadır. Skrotal renkli Doppler ultrasonografide, hipokoik trombüs içeren dilate, komprese edilemeyen pampiniform venler ve venöz akım yokluğu saptanmış olup, bu bulgular pampini-form pleksus trombozu tanısını doğrulamıştır. Hasta antikoagülasyon uygulanmadan, nonsteroid anti-inflamatuvar ilaçlar ve skrotal elevasyon ile konservatif olarak tedavi edilmiştir. Semptomlar iki hafta içinde gerilemiş, birinci ay takip ultrasonografisinde ise parsiyel rekanalizasyon ile birlikte persistan trombüs izlenmiştir. Bu olgu, nadir görülen bu klinik tabloda Doppler ultrasonografinin tanısal değeri-ni vurgulamaktadır.

Pampiniform plexus vein thrombosis presenting as acute scrotal pain

Öz

Thrombosis of the pampiniform plexus is a rare cause of acute scrotal pain and may mimic common surgical emergencies. We report the case of a 62-year-old man who presented with left inguinal and testi-cular pain, normal abdominal ultrasonography and computed tomography findings, and localized ten-derness without swelling. Scrotal color Doppler ultrasonography demonstrated dilated, non-compressible pampiniform veins containing a hypoechoic thrombus and absent venous flow, confir-ming pampiniform plexus thrombosis. The patient was treated conservatively with nonsteroidal anti-inflammatory drugs and scrotal elevation, without anticoagulation. Symptoms resolved within two weeks, and follow-up ultrasonography at one month showed a persistent thrombus with partial recanali-zation. This case underscores the diagnostic value of Doppler ultrasonography in this uncommon entity.
