Ovarian intermediate grade sertoli-leydig cell tumor, with germ cell component?; Case report

Abstract

Sertoli-Leydig Cell Tumors (SLCTs) are rare tumors of the ovaries. They are constituting less than 0.5 % of primary ovarian tumors^and belong to the sex cord stromal tumor group. SLCTs usually seen in the 2nd and 3rd decades. Great majority of the cases are unilateral and confined to the ovary. Histopathology of these tumors characterized by androgen producing testicular structures hence symptoms of virilization can be seen in most patients. Case report; 17 years old female patient with secondary amenorrhea for 4 months and mass in the left inguinal region, was accepted to our hospital. Ultrasonographic examination reveals 60x50x45 mm solid mass in the left adnexial area. Left salpingoooferectomy+pelvic andparaaortic lymph node dissection was performed and intermediate grade SLCT with focal germ cell-like component is diagnosed. Discussion; SLCTs are graded as well, intermediate and poorly according to theirtubular differentiation in the sertoli cell part and the amount of primitive gonadal stroma. Heterologous elements with various differentiation are another feature of these tumors. Endodermal elements as cysts and glandular structures or mesenchymal tissues as skeletal muscle, cartilage and bone can be seen in the 20 % of SLCTs. Also, mixt germ cell-sex cord stromal tumors composed of mixture of germ cells and sex cord stromal cells have been reported. In this report, we present a case of SLCT with germ cell-like component. This rare tumor has been discussed with the review of literature especially for its variants of heterologous components.

Ovaryan intermediate grade sertoli-leydig hücreli tümör, germ hücreli komponent; olgu sunumu

Öz

Sertoli-leydig hücreli tümörler (SLHT) överlerin nadir görülen tümörleridir. Seks kord stromal tümörler grubunda yer alan bu tümörler tüm over tümörlerinin % 0.5' inden azını oluştururlar. Çoğunluğu unilateral, överde sınırlı olup daha çok 2. ve 3. dekadlarda görülürler. Histopatolojik olarak bu tümörler androjen üreten testiküler yapılar ile karakterizedir. Bu nedenle, çoğu hastada androjen miktarına bağlı olarak virilizasyon semptomları görülebilir. Olgu sunumu; Olgumuz, 4 aylık adet gecikmesi ve sol inguinal bölgede şişlik şikayetleri ile başvuran 17 yaşında bayan hasta olup ultrasonografik incelemede sol over lojunu dolduran 60x50x45 mm ölçülerinde solid kitle saptanması üzerine frozen eşliğinde sol salpingoooferektomi ve pelvik+paraaortik lenf nodu disseksiyonu uygulanmıştır. Patolojik incelemede intermediate grade sertoli-leydig hücreli tümör ve fokal germ hücre benzeri komponent izlenmiştir. Tartışma; SLHT'ler sertoli hücre komponentindeki tübüler diferansiyasyon ve içerdikleri primitif gonadal stroma miktarına göre iyi, orta ve az diferansiye olarak grade 'lenirler. Bu tümörlerin diğer bir özelliği de bunlarda bulunabilecek heterolog komponentler ve bunların diferansiyasyon dereceleridir. SLHT'in % 20'sinde kistler ve glandüler yapılar şeklinde endodermal elemanlar ve kemik, kartilaj veya iskelet kası şeklinde mezenkimal elemanlar saptanabilir. Ayrıca germ hücreleri ve seks kord stromal hücrelerinin birbirleriyle karışık olarak izlendiği mikst germ hücre-seks kord stromal tümörler de görülebilir. Olgumuzda yaygın olarak SLHT bulunmakla birlikte fokal alanlarda germ hücre benzeri hücreler görülmüştür ancak immünohistokimyasal çalışmalarla bu hücrelerin natürü belirlenememiştir. Nadir görülen bu tümör içerebileceği komponentler açısından literatür bilgileri ışığında tartışılmıştır.
