The effectivenes of pipelle endometrial sampling in abnormal uterine bleeding

Abstract

Objective: Patient with abnormal uterine bleeding composes 20% of patients applying to our gynecology poly clinics. The most popular procedure for endometrial biopsy in these patients is cervical dilatation and curettage. Recently, more simple, easy and cheap methods are discovered. The most important one is the pipelle endometrial biopsy Material and methods: The study was designed at the Maltepe University Faculty of Medicine, Department of Gynecology polyclinics between March-December 2006.131 women with abnormal uterine bleeding, postmenopausal bleeding, increased endometrial thickness, endometrial polyps, leiomyomas, adnexial masses included to the study. We first performed pipelle endometrial sampling then biopsies were taken with dilatation and curettage. Results: The subject's ages were ranged between 28 and 79 (mean 48±9).32% of the subject were at the postmenopausal period. Premenopausal abnormal uterine bleeding was the front endication for the endometrial sample in the study (28%).9.9% of the subjects were suffering postmenopausal bleeding. Pipelle endometrial sampling was statistically significantly inadequate in the diagnosis of endometrial polyp and hyperplasia compared with dilatation and curettage. Histopathologically 26 subjects were diagnosed as endometrial polyp with D&C. Pipelle endometrial sampling diagnosed only 7 of them. Only 13 of the 21 subjects with endometrial hyperplasia were diagnosed by pipelle. Statistically, there were no difference between pipelle and D&C at subjects with atrophy and endometritis. Conclusion: Many procedures can be used for the diagnosis of anormal uterine bleedings. But the ideal method must be accurate, tolarable, cheap and easy to perform. Compared with many other procedures pipelle fits many of these features. Pipelle endometrial sampling doesn't need anesthesia and can be performed in polyclinics.

Anormal uterine kanamalı hastalarda pipelle ile endometrial örneklemenin etkinliği

Öz

Amaç: Jinekoloji 'de çok sık görülen anormal uterin kanamalar jinekoloji polikliniklerine çeşitli sebeplerle başvuran hastaların % 20'sini oluşturmaktadır. Endometrium biopsisi için kullanılan teknikler arasında en yaygın uygulama alanı bulmuş olanı servikal dilatasyon ve endometriumun full küretajıdır. Günümüzde daha basit, hızlı, ucuz, güvenilir metodlar bulunmuştur. Bunlardan en dikkat çekeni Pipelle ile endometrial örneklemedir. Materyal ve Metod: Bu çalışmaya Mart 2006-Aralık 2006 tarihleri arasında Maltepe Üniversitesi Tıp Fakültesi Kadın-Hastalıkları ve Doğum Polikliniğine anormal uterin kanama, postmenopozal kanama, artmış endometrial kalınlık, endometrial polip, myoma uteri, adneksial kitle şikayeti ile başvuran 131 hasta dahil edildi. İlk etapta hastalara pipelle ile örnekleme yapıldı, takiben D&C ile biyopsi alındı. Bulgular: Çalışma grubundaki hastaların yaş ortalaması 48.3 ±9.3 ve yaşları 28 ile 79 arasında değişmekte idi. Endometrial örnekleme yapılan hastaların büyük grubunu 37 (% 28.2 ) hasta ile pre-perimenopozal anormal uterin kanama grubu oluşturmuştur. Endometrial polip ve kompleks endometrial hiperplazi tanısında, D&C, pipelle tercih edilen gruba göre anlamlı olarak tanı koydurucuydu. D&C yapılan 26 hastaya histopatolojik olarak endometrial polip tanısı koyulurken, pipelle ile örneklemede sadece 7 'sinde benzer tanı koyuldu. Benzer şekilde D&C ile endometrial hiperplazi tanısı koyulan 21 hastaya karşışık, pipelle örnekleme bunlardan 13 tanesinde tanı koydurucuydu. Pipelle ile D&C karşılaştırıldığında atrofı ve endometrit tanısı almışlarda belirgin farklılık saplanmadı. Sonuç: Anormal uterin kanamaların nedenleri birçok yöntemle araştırılabilir. Ancak ideal yöntemin mümkün olduğu kadar doğru sonuç veren, kullanımı kolay, hasta tarafından iyi tolere edilebilir ve ucuz olması gerekir. Pipelle ile örnekleme anestezi gerektirmeden poliklinik koşullarında yapılabilmektedir.
